CHWCMR (Community Health Worker Coalition for Migrants and Refugees) collaborates with CHOICE ACH for care coordination. This partnership aims to reduce health inequities, especially among Latinx communities, by providing integrated care services1. These services include education, social services, medical referrals, housing assistance, behavioral health support, and more
